STEP 1 - Final Forms

Please register in Final Forms and follow the prompts to create your account, create your students and sign your forms. Your athlete will use their school email to login and sign their required forms. Your athlete will also require a sports physical that will be uploaded into final forms.

STEP 2 - Pay Fees

ASB cards are required to participate in any sport. Please log into Intouch to pay for your athlete's ASB card. Pay to Play fees have been waived for the 25-26 season. Your login information is the parents' Qmlativ username and password (Intouch will not open until after August 1). Once the school office opens, you can also pay for the ASB card in person.

STEP 3 - Insurance

Athletes must have medical insurance to participate in sports. If needed, you may purchase accident insurance with Myers-Stevens & Toohey Insurance (insurance for the 25-26 school year will not become available until after August 1).

Participation in Rochester School District’s athletic and activities programs is a privilege that comes with high expectations for conduct, academics, and sportsmanship. The Athletic/Activities Code outlines eligibility requirements, attendance rules, conduct standards, and disciplinary procedures to support student success both on and off the field. Key areas include academic progress checks, adherence to WIAA regulations, responsible use of social media, zero tolerance for substance abuse, hazing, or harassment, and expectations for travel, equipment care, and sportsmanship.
